Mundelein, Ill., Sept. 2015 – Recent data from the Centers for Medicare & Medicaid Services (CMS) suggests that nearly 2,700 hospitals will be penalized for hospitals readmissions in fiscal 2016. Acute and post-acute care leaders are in need of resources to help identify problem areas and improve patient care. Co-sponsoring the summit is Dignity Health, a California-based not-for-profit that operates hospitals and ancillary care facilities in 21 states. More than 200 acute, post-acute care and nurse leaders are expected to attend the summit.
