INTERACT Program Overview

Overview

INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program shown to reduce hospital readmissions by up to 24 percent.1 The program is designed to target three key strategies to help you reduce potentially avoidable hospitalizations:

  • Preventing conditions from becoming severe enough to require acute hospital care
  • Managing selected acute conditions in the nursing home
  • Improving advance care planning for residents for whom a palliative of comfort care plan is appropriate

The INTERACT program includes a state-of-the-art eCurriculum developed in partnership with an interdisciplinary team led by Dr. Joseph G. Ouslander, M.D. at Florida Atlantic University. This online training program gives you all the training and resources you need to implement the INTERACT and safely reduce unnecessary hospital transfers, admissions and hospital readmissions.

Also included in the program is an INTERACT Resource Starter Kit filled with guides, resources and tools.

Online eCurriculum

The INTERACT eCurriculum is comprised of 11 modules covering all aspects of the INTERACT quality improvement program from the perspective of every role involved, including medical directors, administrators, physicians, RNs, NPs, PAs, social workers, LPNs, CNAs, rehabilitation staff, dietary, housekeeping and family members.

User-Friendly Tools

With the program, your facility receives an INTERACT Resource Starter Kit for Long-Term Care that includes:

  • An implementation guide
  • Clinical resources
  • Quality improvement tools

 

 

Dedicated Support

Your facility will be assigned a dedicated program manager who has the knowledge and experience to guide your training, implementation and ongoing evaluation.
